Clinics singled out for optimal care in chronic disease
Affiliated Community Medical Centers and Renville County Hospital and Clinics have been recognized by Bridges to Excellence for meeting optimal standards in diabetes and vascular care in 2008.
Bridges to Excellence was introduced in 2004 to financially reward clinics that meet certain targets in providing care for chronic diseases. In Minnesota, the awards are sponsored by the Buyers Health Care Action Group, a coalition of public and private employers.
Seventeen medical groups in Minnesota and 70 high-performing individual clinics were singled out for this year's awards.
Affiliated Community Medical Centers in New London achieved the standard in diabetes care. ACMC clinics in Willmar and Granite Falls and RC Hospital and Clinics facilities in Hector and Renville were singled out for providing optimal vascular care.
During its first few years, Bridges to Excellence focused on diabetes. Vascular disease was added in 2007 and depression was added in 2008.
