Unconditional bail was set at $15,000 for Emily Katherine Shaffer in Kandiyohi County District Court. Conditional bail was allowed on her personal recognizance.

Her next appearance is March 3.

According to the complaint, Willmar police were called Nov. 2 by personnel at the Family Practice Medical Center regarding someone using multiple names to obtain drugs.

The clinic officials had received information from the local Prescription Center pharmacy that the activity was happening with a prescription for a narcotic pain reliever and an anti-anxiety medication from the clinic.

A pharmacist filling the prescription had recognized Shaffer from a previous prescription and questioned her if her name was Sawicki. She had denied it.

A review of clinic records showed that Shaffer had allegedly given a false date of birth, Social Security number and address when she visited the clinic on Nov. 2. She'd given the Sawicki name, yet another Social Security number and a non-existent Willmar address to clinic officials in June.

Photocopies of the IDs given to the clinic were identified as the same person because they had the same driver's license number.

None of the Social Security numbers matched either name.

During an interview, Shaffer said Sawicki was her maiden name and said she never gives out her Social Security number when she was questioned about the numbers used.
